IN a statement following the CHI findings Jeremy Millar, chief executive of Hereford Hospitals Trust said a detailed plan had been prepared to ensure that the important lesson are learned and failings corrected.

Particular attention was being paid to:

l improving mechanisms for the access to assessment facilities, prior to admission

l means of alleviating the use of mixed sex accommodation

l improvements to IT and information services.

The CHI visit had come at a difficult time for staff who were coping with unprecedented bed pressures, together with the added burden of moving into the new hospital.

The trust was pleased with the praise from the commission and found the report helpful in identifying areas which could be improved further on current good practice.

Mr Millar said the trust was proud of its staff's achievement and the special praise conferred on specific departments.
