NHS Herefordshire Clinical Commissioning Group (CCG) is proud to be supporting the Hereford Times Health and Social Care Awards and to be sponsoring the GP Practice of the Year category.

We believe this is a really positive initiative in celebrating the hard work of NHS and other health and care staff across Herefordshire who work tirelessly to ensure the health and wellbeing of our population.

We hope that staff and patients alike, in this year of NHS 70, will see it as an opportunity to show their appreciation by participating in the awards and sending in nominations and to recognise the dedication of our more than 4,000 staff who work in the health and care sector in the county.

Herefordshire CCG is the NHS body responsible for planning, commissioning (buying) and monitoring healthcare services designed to meet the needs of local people.

The CCG is clinically-led by doctors, nurses and healthcare professionals. It is a membership organisation of 24 general practices looking after a population of 189,300 people.

Through our GP network and wider engagement we continue to talk to local people to monitor our current services and to plan for the future so we can deliver the right health and care services at the right time in the right place for our population.

We also work closely with other NHS bodies, local authorities, Healthwatch Herefordshire and a wide range of other organisations from the voluntary sector. These partnerships are vital to our success.

Our key priorities are to tackle health factors linked to deprivation and health inequality together with continuing support for other core areas, such as dementia, mental health and learning disabilities.

We want to continue to target resources at areas where they will have the biggest impact on improving health outcomes for the people of Herefordshire.
