A 30-YEAR-old died in Leominster after taking heroin.

Adam Kadziewicz was found dead in a locked cubicle in the public toilets in Broad Street by a cleaner at 4.20pm on September 24 last year.

A post mortem revealed he died as a consequence of heroin toxicity.

In a statement read to Herefordshire Coroner's Court, his partner, Anna Kondraczuk said she first met Mr Kadziewicz in 2007 and they moved in together in Leominster the following year.

She first realised he had a heroin problem in 2009. She said Mr Kadziewicz went to rehab in November 2015 and left in July 2016. The following January he went to rehab until May 2017.

They moved to Bristol in July 2017 and returned to Leominster to live in West Street in September last year: less than a week later he died.

Coroner Mark Bricknell recorded a drug-related death.
